GP Shared Care for substance misuse is a new and exciting Service currently being developed in the Borough of Waltham Forest.

An opportunity has been created for the appointment of a GP Liaison Nurse to work within the GP Shared Care Service.

You will be responsible for providing advice, information and guidance on the management of substance misuse patients within primary care in Waltham Forest, providing support to GPs treating substance misuse patients in the Primary Care setting, and working with local specialist drug services to enhance and advance the local Shared Care Service.

You will work within a multi-disciplinary and multi-agency team providing comprehensive patient-centred care packages.

The successful candidate will have knowledge and skills in the assessment, treatment and on-going care of drug-users, be familiar with current issues/trends regarding the management of drug users in primary care and demonstrate a commitment to harm reduction and patient centred care.
