An exciting opportunity has arisen for a General Practitioner with specialist skills in substance misuse wishing to work across the prison and community setting. The postholder will contribute to health improvements in the care of drug users at HMP New Hall and provide clinical input to community substance misuse services locally.
Work will be divided equally between the multidisciplinary community drug team with sessions at the detoxification unit at HMP New Hall, a female prison, where you will take responsibility for the care planning and monitoring of prison receptions. The expansion of the government's Criminal Justice Intervention Programme provides an excellent opportunity for the postholder to support the successful implementation of aftercare services to drug misusing offenders.
As a GP Specialist you will have or be working towards a recognised substance misuse or prison healthcare qualification and be competent in the induction and management of drug detoxification and maintenance programmes. Teaching and training skills will be an advantage.
The postholder will receive excellent professional development opportunities and peer support through a multidisciplinary clinical network. Experience of prison healthcare is desirable but not essential and training will be provided in all aspects of drug and alcohol dependency in accordance with the candidates learning needs.

OVERALL PURPOSE OF POST/JOB SUMMARY
This post is designed for a GP with a Special Interest (GPwSI) in substance misuse who is keen to apply those skills within the prison healthcare setting.
The post combines the provision of clinical services to substance misusers within a community treatment team in the Wakefield District (2 sessions) with clinical support to patients received onto the prison detoxification wing at New Hall female prison(2 sessions).
The post holder will not be required to undertake Out of Hours on call duties. The flexible rota does include evening sessions on the Detoxification Unit.
One of the community clinical sessions will be supporting the Criminal Justice Intervention Through care team and the one session will be supporting one of the voluntary community prescribing teams.
KEY ACCOUNTABILITIES/MAIN TASKS
Service delivery - Prison Sessions
- Provide medical support to the detoxification unit's multidisciplinary team at HMP New Hall including assessment and monitoring of the receptions to the New Hall detoxification wing.
- Undertake and advise on the immediate treatment of patients with drug and alcohol problems using a range of medications and in conjunction with a comprehensive risk assessment.
- The postholder will bring substance misuse specific skills and competencies to enhance the treatment experience of drug and alcohol misusers on the detoxification wing.
- Contribute to the screening of new receptions/transfers at HMP New Hall..
- Accept referrals from prison clinical staff and provide expertise in assessment, treatment, advice and counselling for a range of drug and alcohol related problems in accordance with established prison protocols and procedures and national guidance for the female prison estate.
- Support discharge and aftercare planning for clients who subsequently move off the detoxification wing and into the main prison or upon discharge to the community treatment teams(Ref DANOS AG3).
- Where requested provide medical advice to the Governor at HMP New Hall.
- Where requested provide appropriate reports for the courts, parole and lifer unit.
- Contribute to audit, evaluation and service modernisation within the prison detoxification unit within the clinical governance framework adopted by the prison and WWPCT.
- Assist in education and training initiatives that will raise the awareness of substance misuse and associated problems throughout the prison thereby improving the quality of treatment that patients with substance misuse receive from practitioners and prison staff.
Service Delivery - Community Prescribing (Tier Three, Models of Care)
Undertake the treatment of patients with drug misuse problems using a range of substitute (opiate and non opiate) medications as treatment options.
- Provide clinical and prescribing support to the multidisciplinary community drug treatment teams in accordance with local policies and clinical guidelines.
- Deliver treatment interventions within a criminal justice framework and multi-agency approach that aims to break the link between drugs and crime and acknowledges the importance of partnership working "wrap around care".
- Provide care to patients with complex needs such as homeless, dually diagnosed and pregnant drug users with support from Clinical Director and Nurse Consultant Dual Diagnosis.
- Able to carry out dose induction.
- Provide psychological interventions such as motivational enhancement therapy and relapse prevention.
- Act as a resource to local colleagues operating at generalist level in respect of substance misuse issues.
- Able to signpost patients to other services that will meet their needs in accordance with the objectives within their care plan.
- Ensure that clinical interventions are patient centred and evidence based and in accordance with the principles of safe and responsible prescribing.
- Undertake comprehensive substance misuse assessment and provide advice on clinical management including the direct provision of clinically effective interventions for problem substance misusers. (Ref DANOS AF3, DANOS BC4).
- Maintain effective liaison with other agencies in the field of drug and alcohol including non statutory services (Ref DANOS AH 6.3 and AH 6.4).
- Record and provide information and data as required by the Integrated Substance Misuse Team. (Ref DANOS BE6).
- Devise and manage detoxification programmes in partnership with community drug project workers. (Ref DANOS AH7).
- Maintain up to date knowledge of assessment and management of substance misuse related disorders.
- Provide specialist advice and promote the screening and vaccination for blood borne viruses and promote the messages of safer injecting.
- Provide general medical care for the complications of substance misuse e.g. DVT, cellulites etc.
Facilitate Learning and Development of Best Practice
- Contribute to the learning and development of other practitioners in relation to substance misuse for example through the provision of training sessions in the assessment and management of substance misuse problems.
- Work with other members of the Wakefield Integrated Substance Misuse Team in delivering training in harm reduction, and drugs awareness.
Appraisal and Supervision
- Attend a yearly appraisal interview and fulfil the requirements for revalidation of the generalist and specialist components of the role.
- Access clinical supervision with medical, nursing pharmacy and academic colleagues (Ref DANOS AC2).
- Training and continuing professional development will be integral to the post and in line with RCGP guidelines on minimum standards for GPwSIs in Substance Misuse.
Clinical Audit and Research
- Participate in the audit of activity and clinical standards as part of the clinical governance framework of the Wakefield Integrated Substance Misuse Service This will include monitoring quality and also improving quality where appropriate (Ref DANOS BC4.2 BC 4.3).
- The postholder will be encouraged to undertake research into aspects of their work (Ref DANOS BE3).
Professional and Ethical Practice
- Identify own training needs, through appraisal and clinical supervision.
- Ensure confidentiality and security of written and verbal information acquired in a professional capacity.
- Demonstrate knowledge of ethical issues and their impact on the healthcare of prison detainees and substance misusers.
- Follows professional code of conduct.
Health and Safety and Property Management
- Ensure ones own actions and actions of the team reduce risks to health and safety (Ref DANOS BD3).
- To ensure the protection and best use of property and equipment by observing applicable policies and procedures.

QUALIFICATIONS/SKILLS:
Essential

- RCGP Certificate in the Management of Substance Misuse in Primary Care or equivalent
Experience
- Provision of primary healthcare
- Experience of working in a community drug treatment team (Tier3) or shared care (Tier 2)
