Location: Smithfield Tier 4 In-Patient Drug & Alcohol Service, Manchester

Specification

Turning Point Smithfield Services require input from primary care services that are able to provide clinical leadership and medical services for our 32 bed inpatient drug and alcohol detoxification centre in Manchester. The successful provider will be a specialised generalist/specialist who is able to demonstrate competence and expertise in providing evidenced based medical interventions to clients who have complex health and social care issues.

The services provided will have the following features:

Clinical

To provide a clinical service to people detoxifying on the drug and alcohol units. This should include being able to provide safe, evidence-based interventions to drug and alcohol users using a range of commonly used treatment interventions including:

Pharmacological (e.g. chlordiazepoxide, acamprosate, disulfiram, Pabrinex, methadone mixture, buprenorphine, lofexidine and naltrexone).

Psychological - providing brief interventions and using the opportunity to engage clients with the process of change including advice and information on harm reduction and abstinence in collaboration with the wider programme of social interventions available.

The Provider under this contract will need to see all patients before prescribing commences, although this will normally be following a nurse led assessment.

All patients will need to be registered as 'temporary residents' and will require a full medical assessment as close to admission as possible. (Preferably within the first twenty four hours).

The provider will develop a programme for blood borne viruses, including the provision of hepatitis B vaccination if required.

The provider will maintain written records of all prescribing, assessment and medical interventions for each individual client.

Service development and leadership

The service provider's staff will provide a leadership role to the clinical team, offering support and guidance, ensuring that all medical/clinical interventions are evidenced based and delivered within agreed clinical guidelines and protocols.

We also wish the provider to work with the clinical team to develop pathways of care for clients with drug and alcohol problems, including the development of local referral and treatment guidance.

We expect the provider to participate in clinical team meetings which will include the clinical manager (senior nurse), pharmacist and unit managers on a negotiated basis to discuss clinical and service issues.

The provider should develop clinical audit tools and oversee the implementation of such within the clinical team as a whole, with support from the clinical manager and the unit managers.

The provider will keep up-to-date with developments in the substance misuse and related fields, in order to effect changes in clinical practice.

Liaison

It is expected that the Service Provider will liaise with community based services, primary care and other drug services in the locality as this is integral to the effective delivery of these services.

Where necessary, referral to other health related services will take place.

All patients will also have a discharge summary sent to their own GP and/or substance misuse worker.

Accreditation

We will wish to ensure that the doctors providing these services have submitted evidence of their required competence to the expected standard defined by the Department of Health's criteria which may include accreditation of GPwSI for substance misuse.

We would expect to see evidence of acquisition of competencies by the provider and evidence of maintenance of competencies through at least 15 hours of continuing professional development over the course of a year.

We expect the practitioners to hold a personal development portfolio containing the evidence referred to above.

Administrative support

We expect the service provider to supply administrative support to the GP (s) providing the service.

Facilities to be made available

  • Space for non-clinical work.
  • A clinical examination room with appropriate facilities.
  • Access to appropriate IT systems and support.
  • Access to the Ian Smith Drug Reference Library.

(The provider may wish to have access to NHSnet systems installed, we will facilitate this provision).

Scope of provision

We wish to procure a service to the Smithfield that will provide 9 sessions per week (1 session = 3 hours).

The delivery of these sessions can be negotiated with the provider, however, should be organised in order to meet the needs of the service.

This tender does not include the provision of out of hours/on-call work.

The clinical governance, accountability & monitoring arrangements

The provider will have established clinical governance systems and clinical supervision will be provided through primary care arrangements.

Appropriate arrangements for clinical supervision of the senior nurse will be agreed.

The Clinical Governance arrangements will follow those normally used for Turning Point and include systems or mechanisms for defining clinical audit and communication standards, serious and untoward incident monitoring and complaint handing.

Close co-operation with all parts of the service will ensure that the provider is part of an integrated solution to patient care.

As the service must also comply with CSCI standards, including pharmaceutical regulations, the practitioners must be aware of and comply with these.
