Job Summary
An opportunity has arisen to join the small GP Liaison Team in the Substance Misuse Services based at Lees St in Ashton-under-Lyne. The team currently provides 'shared care' drug treatment interventions to approximately 200 patients in 11 primary care clinics across Tameside and Glossop.
Our partnership work involves a broad range of both statutory and non-statutory agencies but in particular GPs and other primary health care team staff, and community pharmacists.
The successful candidate will be involved in the regular collaborative assessment, treatment and review of relatively stable patients in a number of clinics. In addition, the candidate will identify patients within the caseloads of core drug team colleagues who are suitable for primary care, and manage the transition of these patients towards shared care.
A nursing, social work, or equivalent qualification, or 3 or more years experience in the substance misuse field is essential.
The post involves being a specialist drug treatment resource to other staff, especially GPs. The post holder will be able to work effectively under pressure within a broad motivational enhancement approach. The ability to work autonomously, and in a self-directive and proactive manner is crucial, as is the willingness to be flexible and responsive in covering the needs of the service as a whole.
The role will take the successful candidate into close contact with the environment of primary care, and offers an opportunity to experience service delivery in the context of a local drug treatment system that is recognised by the National Treatment Agency as being one of the most efficient in the country.
The service provides a high level of support and supervision both formally and informally.
