Job title: Clinical Nurse Specialist - Substance Misuse
Term: This post is for a term of one year, with the possibility to extend.

In partnership with the City of London DAT, City and Hackney teaching Primary Care Trust is recruiting a shared care nurse practitioner required to lead on the provision of care to the homeless which includes those vulnerably housed and substance misusers. The City of London Drug Action Team (DAT) has the responsibility of providing a specialist substance misuse treatment service in The City of London.

You will be required to work across two sites and two services:

  • Health E1 - a nurse led homeless PCT medical service
  • The Neaman Practice - a general practice in EC1

You will work autonomously and collaboratively with nursing colleagues, GPs, practice team members and staff at the DAT and within Health E1, carrying out comprehensive nursing assessments and devising (in partnership) care plans/pathways for patients.

We are therefore seeking a highly motivated, skilled and competent nurse with at least 3 years post qualification experience in a community setting and/or experience of working with homeless persons or with substance misuse. You will ideally possess advanced nursing skills, be a critical thinker with the ability to use sound clinical judgement when dealing with complex issues. You will also possess the capacity to be a work in a challenging but stimulating environment and have a clear understanding of current legislation, polices and best practice guidance (locally and nationally) enabling you to provide a high quality service.

The post holder must demonstrate enthusiasm and drive, together with an appreciation of the priorities of the City of London DAT and how these work with the priorities of the PCTs for the benefit of this client group.
